WAIVER AND RELEASE OF LIABILITY
In consideration of the opportunity for my child to attend the Kannada Kali school (“the Classâ€), I, the undersigned, assume all risks associated with my child's attending and participating in the Class, and waive, release, and agree to hold harmless the Kannada Kali, and its agents, Principal, Vice Principal, Treasurer, Secretary, Joint-Secretary, Teachers, Volunteers and Student Volunteers, BAYVP temple authorities, Sunnyvale middle school authorities, American high school authorities, and KKNC from any liability to the undersigned, and to the personal representatives, heirs, assigns, and family of the undersigned, for all loss or damages on account of injury to the person or property of the undersigned and undersigned’s child relating to attendance at or participation in the Class, Choir, or School Day, including any claims of damage or rights of action arising out of the use of any premises for the Class, Choir, or School Day, whether the alleged injuries or damages arise from the negligence of any of the releases or not, to the fullest extent allowed by law. This release includes, without limitation, any claims arising in negligence, other tort, or contract.
The undersigned has read the Waiver and Release of Liability, understands its terms, understands that by signing he or she is giving up certain rights he or she might otherwise have under law, and voluntarily signs.
By signing this form, parents/guardians agree to this waiver, give permission for their children to participate in the Class at BAYVP Temple, Milpitas, Sunnyvale middle school, Sunnyvale, American high school, Fremont and Invocation at KKNC event and School annual Day per the Rules and Guidelines that follow, and give permission to the adult in charge to seek emergency medical treatment and/or transportation for their children.
Media Release: I hereby grant permission to Kannada Kali and its affiliates to use photographs, videos, or other likenesses of my child taken during Class, Choir, School Day, or related activities for promotional purposes in newsletters, Facebook, and other social media platforms.
